I'm a board-certified internist and pediatrician practicing primary care, and I spend part of my week as a physician informaticist helping optimize the electronic health record for clinicians and patients.
For years, my wifeâalso a primary care physicianâand I sat at opposite ends of the couch on weeknights and weekends, laptops open, closing charts and clearing inboxes. Nobody trained us to document efficiently under real clinical conditions. Medical school didn't teach it. Residency modeled the opposite. We were told the after-hours work was just the job.
It isn't.
The turnaround for me wasn't a new template or a faster computer. It was workflow changes andâharderâunlearning the beliefs medicine had installed about what a note has to be. Small adjustments compounded. I started getting home with charts done. Colleagues noticed and started asking how.
That became Charting Conquered: a six-week program combining structured workflow training with the behavior-change work that templates and scribes can't touch. Since 2021, I've coached hundreds of physicians and APPs through it across every kind of outpatient setting and EHR.
Because I'm an informaticist by training, I measured it. Across 130 participants with paired pre/post data, after-hours charting fell by halfâ6.7 hours a week reclaimedâburnout dropped 39%, and professional fulfillment rose 21% on the Stanford Professional Fulfillment Index. My goal is to help 10,000 physicians and APPs eliminate after-hours charting â over a million hours of personal time back where it belongs.
If you're still charting at home, you're not inefficient and you're not broken. You were never taught this. That's fixable.
